External Partner Questionnaire
Gather consistent information from founders, academics, consultants, and external partners with this public questionnaire template. Four tabs collect personal, project, relationship, collaborator, supporting-document, and declaration details without requiring a Microsoft 365 account. Dynamic security-question lists prevent duplicate selections, and collaborator data is stored in a structured table.
This template is inspired by a customer story from the University of Manchester Innovation Factory, which uses public Plumsail Forms to collect information from external users and pass it into SharePoint-based processes.
Structure and logic of the form

The questionnaire uses the Tabs control. External users can move between the profile, project, security, and declaration sections in any order without a Microsoft 365 account.

Additional collaborators and founders are collected with the Data Table control. Each row stores a separate name, role, email, and organization for structured workflow processing.

The three security-question dropdowns start with the same list of ten questions. Once a question is selected, JavaScript updates the other fields using the Drop Down widget's setDataSource method, preventing the same question from being selected twice.

The submitter's name is filled from the first and last name fields, and the submission date is populated automatically.

Supporting files are collected using the Attachments field.
