Worker Site Check-In Form
Use this mobile-friendly check-in form to register workers and contractors as they arrive at a site. Collect contact, employer, work-purpose, and site details, add the check-in time automatically, and optionally capture the phone's current location. Create site-specific QR codes by passing the location name in the form link. Share the public form without requiring an account and connect submissions to attendance, safety, or site-management workflows.
This template is inspired by a customer story from our partner KKiT, which designed a public form for mobile site check-ins that captures location data directly from the user's device.
Structure and logic of the form

You can share the form via a QR code, which can be generated in the form settings.

The check-in form uses a compact, single-column layout built with Grid containers.

The Mobile phone number field accepts valid international phone numbers. In the field properties, you can select allowed countries.

The check-in date and time are filled in automatically when the user submits the form.

The Site / location (based on IP) field is populated using the ipapi API.

After submission, users see a confirmation that they have checked in successfully. You can customize the message in the form settings.
