Contractor Site Induction Form

This template is inspired by a customer story from our partner KKiT, which designed a public form to collect site induction information from contractors and workers.
Structure and logic of the form

The site induction is divided into three focused steps using the Wizard container: worker details, work details, and safety sign-off.

When a user selects electrical work, hot work, scaffolding, lifting operations, heavy equipment operation, or confined-space work, the Relevant license or certificate upload appears. This is done with conditional rules in the Hide property of the field.

Workers can upload proof of competency using the Attachments field.

The four safety acknowledgements are required. Users must confirm that they understand site rules, emergency procedures, PPE requirements, and hazard reporting before submitting the induction.

Workers complete the sign-off with an electronic signature.
