This template is inspired by a customer story from Laneway OT, an occupational therapy provider that uses Plumsail Forms for service agreements, client intake, and assessments with conditional fields and electronic signatures.
Structure and logic of the form

The agreement is split into five steps using the Wizard container. This keeps client, service, funding, representative, and agreement details manageable on mobile devices.

Home-visit address and access fields appear only when Home visit is selected. Additional service questions appear for counselling, clinical consultation, and home healthcare services. This behavior is configured with conditional rules in the Hide property of Grid containers.

Representative details remain hidden unless someone is completing or signing the agreement on the client's behalf. The signature guidance also changes to identify whether the client or representative is signing.

The client address and the client or representative name are reused automatically in the billing and signature sections. Users can edit the copied values without losing their corrections.

The four agreement acknowledgements are required, and the agreement can be completed with a signature.
