IT Help Request Form
Using the IT Help Request online form, customers can instantly report any technology request or problem they experience. A customer needs to fill out all the general information such as name, email, department, and select the type of request and share its details.

Fields and controls
- The Email field accepts only valid email addresses. Change the pattern in the field properties.
- Change options for the Department and the RequestType fields in the field properties.
- Change the message that will be displayed after the form is submitted in the form settings.
- The form timeout is set to 60 seconds, then the form can be submitted again. You can change it in the form settings.
Styling
- The primary button's font-weight is set to 700 via the Style property.
- Styles for h1 is defined in the CSS-editor.
- A pseudo-element is added after h1 element to display double lines.
